Visitors must provide the following documentation to receive honorarium or reimbursement pay from Mount Holyoke College:
Honoraria for services performed in the US is subject is subject to 30% IRS withholding unless a treaty is applicable between the foreign visitor's country of residence. To claim treaty benefits, the visitor must have a SSN or an ITIN, and provide travel history.
For travel reimbursement: All expenses must be supported with receipts.
Departments must send a Check Request to Accounts Payable at least one-week before expected payment to visitor. AP will not process the payment until the following documentation (by visa type) has been provided to Financial Services:

B1 - B2s: (Visitor for temporary business) Visa Waiver or ESTA:
Copy of Passport
Copy of Visa / ESTA Form (Visa waiver countries do not have a visa page. In absence of visa, provide the ESTA form)
Fill out form: “9/5/6 rule” form
Fill out form: Affidavit for visitors without SSN (only if visitor does not have a SSN or ITIN)
Fill out form: W-8 BEN Form
Fill out form: Travel History Form (if not doing it through FNIS)
F-1 Visa (CPT or OPT) :
Copy of Passport
Copy of Visa
Copy of I-20 Form
Fill out form: Affidavit for visitors without SSN (only if visitor does not have a SSN or ITIN)
Fill out form: W-8 BEN Form
Fill out form: Travel History Form (if not doing it through FNIS).
For OPT: Copy of Employment Authorization Document (EAD)
Requires letter from visitor’s sponsoring F-1 visa institution.
J-1 (Exchange Visitor Program) :

Copy of DS-2019 Form
Fill out form: Affidavit for visitors without SSN (only if visitor does not have a SSN or ITIN)

Fill out form: Travel History Form (if not doing it through FNIS).
