ENROLLMENT AND GRADE RELATED FORMS
To enroll number of units/subjects more than the prescribed number in a current semester. Applicable only for students in their terminal semester.
To take two or more pre-requisite subjects in one semester that are originally offered in different semesters and school year. If failed in one subject, automatic in all simultaneous subjects applied. Applicable only for students in their terminal semester.
To add or delete officially enrolled subjects. Period of request is only within the first two weeks from the start of classes.
To request for manual enrollment beyond the prescribed period of enrollment. Late payment fees will be charged accordingly.
To file for suspension of enrollment/study.
To request to be included in a class that is already closed due to its capacity. Subject to the approval of the faculty and Program Head.
To request for readmission after suspension of enrollment/study without filing an official leave of absence.
To request to open a class that is not offered in the current semester. Student/s enrolling must agree to pay the entire amount of the class.
To shift from a current course to another course. It can be in the same or to another college. Subject for crediting and evaluation.
To file for official re-enrollment/ continuation of study from LOA.
