Transcript Request Information for Alumni & Schools/Organizations
Education Verifications & Transcript requests should be sent to transcripts@mpspride.org
Schools/Organizations, please email a signed release of information form with the student’s full name while enrolled, graduation year, and date of birth.
Former students, please send a copy of your driver's license or state issued ID, full name at the time of graduation and the graduation year.
If sending transcripts to a school or organization, include their contact details (i.e. address/email to be sent to).
Personal copies will be marked unofficial. This request can take up to 3 business days, follow up calls are not necessary.
Transcript Request Information for Current Students
Current MHS students must complete the TRANSCRIPT RELEASE FORM to have their transcripts sent to any post-secondary institution or scholarship program.
