Mandated Health Screenings and Physical Examination Requirements


The following state-mandated screenings are conducted annually by the school nurse. Screenings support academic achievement by early identification of conditions that may impact learning. Screenings are conducted individually in a confidential manner. Parents and guardians will only be notified if the screening requires further evaluation by the student's health care provider. Parents and Guardians may opt the student out of the screening by providing a written/email opt-out request prior to the screening. It is then the expectation that the student's health care provider will be responsible for completing the screenings. Current physical examinations by the student's health care provider are required at designated intervals and more frequently for athletic participation. (See Athletic Health Requirements). Documentation of the current physical examination should be provided to the school nurse.

*Height and Weight- Grade 7, Grade 10
