Epinephrine Forms Checklist- Click here to access the forms required for the nurse to administer Epinephrine to your child, in the event of an emergency, during the school year.  All forms can be found below and accessed by clicking on the form, which will direct you to the link.  All forms are required prior to the start each new academic school year. Medication must be brought in, by the parent, prior to the first day of school.

IMPORTANT NOTE: Medication will not be accessible for use by any outside groups.  It will be the responsibility of the parent/guardian to provide an additional supply of medication to any non-school sponsored activity/outside group that the child is participating in.  For example, but not inclusive of; HSA Sponsored Activities, YMCA before-care/after-care, boy scouts/girl scouts, and recreational programs.
