Pre-approval requests must be submitted at least 4 WEEKS prior to payment date for requested funding. Requests over $10,000 requiring a contract may take 8-12 weeks; please plan accordingly.
The online pre-approval request form must be completed in one sitting. You will NOT be able to save your form and go back to it. Please have all necessary information for your request before you begin (see below).
You will be able to download and save a PDF copy of your request. After you submit the request, the next page you will see is the completed request and a link that says “Download PDF.” We encourage you to save a copy of your requests.

Basic conference information (title, sponsoring organization, city and state of conference, start and end dates).
A description of the conference/workshop (you can upload a screenshot of the conference/workshop website). Nonpublic school staff should NOT register for the conference themselves.

If this request is for more than $3,500 you need to provide 3 quotes for the requested services.
Names, email addresses, and job titles of your staff who will be participating in the PD activity
Specific questions you will need to answer:
Please describe the proposed PD activity.
Address the following: the learning and activities expected of the provider/staff; the duties to be performed by the mentor/staff/consultant; types of services provided; how the contractor/provider will deliver the services (e.g., direct instruction, tutoring, workshops, professional learning communities, consultation, observations, online, etc.)
If the activity is not secular, neutral, and non-ideological, can it be clearly separated so that the district only supports the secular portions of the activity? If no, this PD request cannot be approved.
What are the overall goals of the PD activity/services to be delivered?
Goals should show a connection to: the services and activities described above; how the PD activity will impact student learning; how the learning/activities will be shared with the school community
Describe the methods for evaluating the effectiveness of the services to be provided.
This information must include: what methods (ex: survey, observation, evaluation of student data, etc.) will you use to determine the extent to which you reached your goals outlined above; when a follow-up evaluation will be conducted and by whom
How does this activity fit into the overall PD plan that has already been agreed upon?
If the activity involves travel (ex: an out-of-town consultant), is there a comparable resource locally?
If you're submitting a request that includes contracting for services from a third party provider or consultant...
Provide the following information:
Name, address, email, and phone number of provider/consultant
Party responsible for signing the contract
Name and title of nonpublic school staff recommending the contract
Resume of the consultant OR the credentials of the providing organization
Any pertinent information regarding the costs of the contract

Completed Supplemental Supplies and Materials Order Request Form, which you'll upload to the online pre-approval request form
Includes title and description materials, the number of each material you are requesting, cost of materials
