All orders must be updated for each school year and any time a change is made to the students medical plan of care.
A written Authorization for Administration of Medication listing medical diagnosis must accompany each medication to be dispensed. No medication is administered by the health office without specific written instructions from a medical provider. This includes all over the counter (OTC) medication except as noted below.
Both parent/guardian and prescribing health care provider must sign the authorization sheet for prescription medications given during the school day by the health office.
Each of these forms can be obtained from your student's health care provider and must be signed by the health care provider and parent/guardian.
Only applies to HIGH SCHOOL students
Must comply with MN Statute 121A.222
Parents/guardians of HIGH SCHOOL students ONLY in compliance with MN Statute 121A.222, must complete the Authorization for Self-Administration of NON-Prescription Medication form if they'd like their student to carry/self medicate with OTC medications like Tylenol, Advil etc. No combination medications or medications containing Pseudoephedrine may be carried by the student. Parent/guardian and student signatures required.
At times, it is appropriate for prescription medication to be kept with the student in the HIGH SCHOOL once approved by the school nurse with proper medical provider orders. Examples of these instances include Asthma, Allergy/anaphylaxis, Seizure, Diabetes. Complete the Self Carry/ Self Administration Prescription Authorization. Medical provider, parent/guardian, and student signatures required.
NOTE: If your student is prescribed narcotics, the school recommends keeping them home until over the counter pain medication is sufficient to manage their pain.
Consent for treatment.
Please provide Treatment Authorization for any non-medication related medical need your student may have (requires medical provider signature).
Late start/early out.
Please provide instruction for the health office for your student's medications and treatments on late start or early out days.
