The Minnesota Department of Health encourages families to continue getting immunizations for their students during the pandemic.
The Minnesota School Immunization Law (Statute 121A.15) requires students in Preschool through Grade 12 to be adequately immunized or show proof of legal exemption before starting school.
In order to enroll your student(s), parents or guardians must provide a written immunization record for their student(s). The Are Your Kids Ready? chart has been developed by the Minnesota Department of Health and updated on August 2024. This chart shows the immunizations required for children in Minnesota schools by age.
Use this Immunization Form to document vaccines as well as medical or non-medical exemptions.
Be sure to update your student's health file by sending written verification each time he or she has received another immunization.
Immunization Exemptions are accepted. Parents/guardians may file a medical exemption signed by a health care provider or a conscientious objection signed by the parent/guardian and notarized. These will be kept on file at the school and must be updated upon entering Kindergarten and Grade 7.
The meningococcal (MenACWY) booster dose is required for all 12th grade students. Generally, students should have received the first dose of MenACWY vaccine prior to seventh grade, followed by a booster dose when the student turns 16 years of age. The practical implementation for the school requirement is as follows:
Grades 7 through 11: One dose
Grade 12: Two doses
