CFCC Updates (4/25/23)
Both imaging and non-CFCC specialty referrals are now done by the central team. Major changes to note:
Do not send phone notes to Ravi or Theresa about referrals issues. We do not need to send phone notes when we order CT or MRI.
If you receive any messages about referrals (e.g. "I need a HIP referral") then use the smart phrase ".referralcoordinator" to figure out who to route the phone note too. (these are the referral coordinators on the central team).
For open MRIs, write it in note and have patient request it when they call to schedule
For breast imaging forward to Cheryl Gooden and cc Shareen Pradier and Nicole Allen with".breastdiagnosticimaging" dotphrase
If patient missed imaging and needs to reschedule message Jamesa Cooke and Ana Villodas and cc Shareen Pradier and Nicole Allen
PGY-3s: Please start working on your “high risk” lists by making a list as you see the patients. Try to write an excellent note that would make it easier for the next doctor to understand the complexities of the patient. Next block, we will ask that you share these lists with your team attending, myself, and Juan Hernandez.
Involve LPNs for inbox management: try to send at least 1-2 phone notes to the LPN on your team asking them to follow up on a lab. Label the phone note “results.” They can only follow a script, so use the smart phrase ".labnurse". Please ask your box buddy or another attending for guidance if needed.
For CFCC specialty referrals AND non-CFCC specialty referrals, please put the referral in the system.
Our CFCC-specialty referrals will still be scheduled by the front desk PSR, BUT someone will also manage the work-queue to be sure to catch any that did not get scheduled.
Can use “route to coordinator” button to be sure the patients are called for appointments.
Med PAs: we are having trouble with these. Please include Dr. Harris, Celena Dublin, and Juan Hernandez on the request. Please be sure to send in a phone note labeled “prior authorization” (change the Rx Request to a phone note).
During visits, please utilize the After Visit Summary: Click the box that says when to come back and use check out note for labs/specialty visits
Referrals: please route requests for diagnostic mammograms (e.g. for after a birads 0 mammogram) to your referral coordinator though a phone note.
Shingles vaccine: Patients with Medicare (of any age, even if also have Medicaid) cannot get these done directly in clinic. If your patient has Medicare you need to send it to the pharmacy.
The LPN for your team will do employment or school forms. If a patient brings you a work form during a visit, please look at and order what may need ordering (e.g. quantiferon, titers, vaccines), and give to your LPN so they can follow the workflow. Make sure all the forms you do need to do (still M11Q, CDPAP, FMLA,etc) are reviewed and signed by an attending.
