  1. Go to https://forms.moe.edu.sg/ and Log In

2. Click "NEW FORM" 

3. Select the Audience tab

4. Then, select your Audience Type

For the Public audience type:

5a. Proceed to create the form under Question. For instructions on setting up questions, go to Setting Up Questions.

For Students' audience type:

5a. Proceed to Add individual or groups for the selected groups

For the Staff audience type:

5a. Proceed to Add fields to personalise for the selected groups
