Status Change/Withdrawal

Mansfield Dive Club Status Change

Members wanting to Withdraw from the club must fill out this Status Change Form and email it to Andy Serie. A written request must be provided to withdraw no later than the 20th of the month or your account will be charged for the following month on the next billing cycle which is the 1st of each month.

Payments will stop on the first of the month of the next billing cycle (1st of each month).

Reinstatement Fee of $100.00 per child if requested within a 6 month period.

Medical Inactive Status:

Parents can request a hold on their account for extended medical needs.
