Because of Coronavirus, UIL has modified the rules for having a completed physical.
1. Anyone new to Midland ISD athletes (7th grade or new) must complete a full physical.
2. Everyone must complete the online medical history form on RankOne. If you answer "Yes" to any of questions 1-6, you must get an updated physical.
All athletes must have these done by the start of the first practice or for tryouts - whichever comes first.

This form is used when a parent wishes to pickup a student after an out-of-town competition
(PAPF)
If you are transferring to a new school, you must complete this form and get it approved before you are allowed to compete for your new school.
Some transfer paperwork may require the following Parent Address Waiver.
