Forms

Form are available for Midland MS/HS educators, parents, and students to request assistance for students they feel will benefit from a positive behavior intervention. Once a form for a student is submitted, it will be reviewed by the PBIS team members. When the PBIS team makes a decision, they will be in contact with the individual making the recommendation. Depending on the decision, the intervention for the student will be put into place within one to three school days.
