MIDDLE TOWNSHIP ELEMENTARY #1 SCHOOL

2023-2024 PRESCHOOL & KINDERGARTEN

REGISTRATION

Registration for full day preschool and full day kindergarten will be held at the Middle Township Elementary #1 School, 215 Eldredge Road, Cape May Court House on, MONDAY, MAY 1 thru FRIDAY, MAY 5, 9:00 AM to 3:15 PM, by appointment only. Parent/Guardian must pick up a registration packet prior to registration, at that time you will receive your appointment. Packets are available for pick up at Elementary #1’s main office. No appointments will be made by telephone. If your child is already enrolled in our preschool program, it is not necessary to attend this registration.

Preschool students must be 4 on or before Oct. 1, 2023 and Kindergarten children must be 5 by that date.

NO STUDENT WILL BE CONSIDERED REGISTERED UNTIL ALL OF THE FOLLOWING PAPERWORK IS COMPLETE AND SUBMITTED.

Registration must be made in person by parent or guardian only.  Parent/guardian must bring the following paperwork:

1.      Original birth certificate (Baptismal & hospital certificates are not acceptable)

2.     Proof of address as follows: #1 (one) Property deed; tax bill; or contract of sale; Signed Lease; #2 (two) Recent utility bill (last 2 months only); Bank statement (last 2 months only); Public assistance documents (last 2 months only); Voter registration.

3.     Photo ID with current Middle Township address is required from parent/guardian for registration; this cannot be used as a proof of address.

4.     Legal papers if applicable – (eg: custody, restraining order etc)

5.     Current Physical

6.     Immunization requirements:

 DPT – series of 4                                     DPT –series of 4 with one given after age 4

 POLIO – series of 3                             POLIO -series of 3 one given after age 4

 MMR – 1 dose after age 1     MMR – 2 doses after age 1                                                                  

 HIB – 1 dose after age 1                          HEPATITIS B –series of 3     

 HEPATITIS B – series of 3                 VARICELLA – 1 dose after age one

 VARICELLA (chicken pox)-1 dose after age one   

 INFLUENZA – Annual in the fall                         

    PNEUMOCOCCAL – 1 dose after age 1
