A Physical Form or an Alternate Year Physical Form is required to be submitted to the Middle School front office before an athlete is allowed to practice.

A $30 Athletic Fee is due to the Middle School front office per athlete.

The Concussion Form must be submitted to the Middle School front office.

A Travel Release form must be completed ahead of time and approved by the principal for any student-athletes not riding the bus home from meets.
