Over the summer, we were tasked with completing 8 case studies in addition to your clinical site work. These case studies were created through the EHRGo platform we used in the spring during MNT. Each patient had a different condition, some chronic some acute. We were tasked with navigating their medical records, writing nutrition assessment notes, coming up with a nutrition care plan, and even calculating enteral and parenteral nutrition prescriptions regardless of whether the patient technically needed it or not.
In this example, the patient was 1 day post-op bariatric surgery.
During my time at Lawrence General Hospital, I followed a complicated patient through his stay which involved a trip to the ICU. This document includes my notes after each encounter I had with him. This particular patient presented to the hospital with a recurrent syncopal episode and had a history of several GI disorders leading to a partial gastrectomy that was complicated by candidial endocarditis. He was found to have an esophageal stricture that was barely wide enough to let thin liquids through. He had lost almost 40 pounds in 3 months due to dysphagia and not being able to eat! We put him on PPN in hopes of replenishing some nutrients for him without causing refeeding syndrome. On 8/22/22, the patient went into surgery for an additional esophageal dilation. As of 8/23/22, the patient unfortunately left the hospital against medical advice.

These assignments included navigating a patient's medical record and writing an initial nutrition assessment note that included a nutrition care plan. Along with the note, we also calculated EN/PN needs for each patient.
