Students and Parents, 

Being injured or sick can be a difficult time for any athlete. The MCHS Sports Medicine program would like to make this process a little less stressful by providing some useful information regarding injuries and illness. As always we are available to answer any questions the athletes, parents and coaches have in these areas, so please feel free to contact us with any questions you may have.

Responsibilities of the
Student- athlete

In order to achieve the objective of quality medical care, each student-athlete has the responsibility to:

Pre-Participation Examination Form:

The VHSL Pre-Participation Examination (PPE) form is needed for all students to be able to participate in athletics. It must be completed and signed by both the student and a parent/guardian. It requires an examination by a physician, also signed and dated AFTER May 1 of the subsequent school year. 

Please use the following form; you CANNOT use an out of date form (i.e. "Revised February 2017" seen in the top right corner).
