Students and Parents,
Being injured or sick can be a difficult time for any athlete. The MCHS Sports Medicine program would like to make this process a little less stressful by providing some useful information regarding injuries and illness. As always we are available to answer any questions the athletes, parents and coaches have in these areas, so please feel free to contact us with any questions you may have.

In order to achieve the objective of quality medical care, each student-athlete has the responsibility to:
Be honest and direct with the athletic trainer(s) caring for you.
Report all injuries and illnesses within 48 hours.
Follow the treatment and rehabilitation plan prescribed by your physician or athletic trainer. It is your responsibility to advise the athletic trainer if there are any problems or reason why you cannot follow the plan.
Report to the Athletic Training Facility daily, or as directed, for treatment and rehabilitation until you are cleared by the athletic trainer or medical doctor.
Report any change in your condition to a staff athletic trainer
Understand your health problem and the treatment and rehabilitation program to your satisfaction. If you do not understand any part of your treatment/rehabilitation program, ask a staff Athletic Trainer or physician. Be sure you know what is expected of you.
Pre-Participation Examination Form:
The VHSL Pre-Participation Examination (PPE) form is needed for all students to be able to participate in athletics. It must be completed and signed by both the student and a parent/guardian. It requires an examination by a physician, and is valid for 14 months after the date of exam.

Annual Concussion and Health Education:
All student-athletes and their parent/guardian must complete the Annual Concussion and Health Education form. This ensures that students and families have the current information needed to practice and play safely on MCHS/MCMS campus. It only needs to be completed ONCE per school year before the start of the student-athlete's sports season.
