FOR STUDENTS IN GRADES 5TH - 8TH ALL BUS FORM CHANGES MUST BE MADE AT MAYFIELD MIDDLE
*For middle school students, bus drivers are no longer required to see the student enter their home. It is the responsibility of the parent or guardian to make sure their child is able to enter their home upon delivery by the bus.
FOR ANY ADDRESS CHANGES THAT NEED TO BE MADE: The transportation forms MUST be filled out and turned in by a parent or guardian at school. If the form is not filled out and turned in by a parent or guardian, any and all changes will be rejected. These forms should be turned in before 1:00 p.m. in order for the new changes to take effect the following day.
IN CASE OF AN EMERGENCY: If there is an emergency (in the hospital, broken leg, car is broken down etc.) and the student needs alternate bus transportation, a parent or guardian must call Mayfield Middle School to request a change which must be approved by the principle.
