FOR STUDENTS IN GRADES K - 4TH ALL BUS FORM CHANGES MUST BE MADE AT MAYFIELD ELEMENTARY
*It is the responsibility of the parent to make sure your child is able to enter their home upon delivery by the bus. If the student fails to get into their home, the student will be taken back to M.E.S. and it is a parents/guardians responsibility to pick up the child at school. Failure of a child to get in the residence could result in suspension of bus privileges.
FOR ANY ADDRESS CHANGES THAT NEED TO BE MADE: The transportation forms MUST be filled out and turned in by a parent or guardian at school. If the form is not filled out and turned in by a parent or guardian, any and all changes will be rejected. These forms should be turned in before 1:00 p.m. in order for the new changes to take effect the following day.
IN CASE OF AN EMERGENCY: If there is an emergency (in the hospital, broken leg, car is broken down etc.) and your child needs alternative bus transportation, a call must be made by a parent or guardian to Mayfield Elementary with an alternate plan which must be approved by the principle.
