HEAD START RULES AND REGULATIONS for Bus Transportation
ALL BUS FORM CHANGES MUST BE MADE AT MAYFIELD ELEMENTARY
*If a parent/guardian fails to show the proper transportation card or fails to be present at the time the child is delivered, the student will be taken back to M.E.S. It is a parents/guardians responsibility to pick up the child at school. Failure of a child not being able to be delivered could result in suspension of bus privileges.
FOR ANY ADDRESS CHANGES THAT NEED TO BE MADE: The transportation forms MUST be filled out and turned in by a parent or guardian at school. If the form is not filled out and turned in by a parent or guardian, any and all changes will be rejected. These forms should be turned in before 1:00 p.m. in order for the new changes to take effect the following day.
IN CASE OF AN EMERGENCY: If there is an emergency (in the hospital, broken leg, car is broken down etc.) make sure that an adult 16 years of age or older is at the students delivery address with the proper transportation card. Two cards are given out at the beginning of each year, it would be best to keep a transportation card with you and one card in a safe place where a trusted person could have access to it if an emergency should arise.
