Step 1: Set up a new FORM

Step 2: Edit the FORM into a QUIZ (at the top right, click settings).

Step 3: Click "Quizzes", and choose the options. (Important: always click SAVE -bottom right-).

Step 4: Add questions.

Step 5: To the right of the question title, choose the type of question you want. Type the possible responses to your question. To prevent people from not answering, turn on Required.

Step 6: There is the option of PREVIEW your quiz before sharing or sending it. Click the preview button at the top right.

Step 7: And remember, you have always the option of "Docs Editor Help"

Step 8: You can check all the steps you have to do, and there is a Help Forum too. (What else do you need to start and learn by yourself? Enjoy it)
