Spring Credit Flex Application - Open!
Students may earn graduation credit by generating and developing an Individual Plan for any of the following options: independent study, internships/field experiences, or educational travel. This also includes a Physical Education credit flex plan.
*Please review ALL information in the Credit Flexibility Handbook.
*Students must submit a Credit Flex application and wait for approval prior to beginning the Individual Plan and accumulating hours towards the Individual Plan. Retroactive credit will not be awarded under any circumstances.
*Students are responsible for all fees or supplies necessary to carry out an Individual Plan.
*The following elements must be addressed/discussed in the Credit Flex application/plan submission:
All Individual Plans require a mentor. Mentors cannot be related to the applicant.
The application/plan must address all 5 Physical Education standards and benchmarks for the appropriate grade band (middle school 6-8, high school 9-12)
*What do I turn in when my plan is complete?
PLAN SUBMISSION COMPONENTS: The following elements must be submitted to the Credit Flex counselor or administrator upon completion.
A log of hours demonstrating the type of activity, date, and duration of time spent engaged in the activity. This log must be signed off by your mentor verifying that you completed the activities specified in the log.
**A minimum of 60 hours of physical activity is required to earn 0.25 credit.**
There is not a template provided for the log of hours - students may use whatever format they prefer (Google Sheets, journal, etc).
A written reflection and/or evidence that addresses how the student's Individual Plan meets the 5 PE standards listed below. More detailed information on the State of Ohio PE Standards can be found here: 5 physical education standards and benchmarks.

*Based on the details on the Individual Plan, students may be required to make a formal presentation to a committee that is determined by Mason Middle School, which can include, teachers, staff members, and/or administrators.
