Please complete all sections of this questionnaire and upload below signed hard copies (electronic signatures are not accepted) of both the Intent to Participate Form and the CCP Counseling Session Participation Form to officially confirm your intent to participate in the CCP program for the 2026–2027 school year.  

26-27 College Credit Plus Intent to Participate Form

We will not be accepting intent forms prior to this date

 This form must be completed by 11:59 p.m. on April 1, 2026 to participate in the CCP program during the 2026–2027 school year. If the April 1 deadline is missed, the intent form may be submitted by November 1, 2026 for Spring 2027 eligibility only.

Counseling Session for CCP Participation

This form must be completed and submitted by April 1, 2026 to participate in the CCP program at any time during the 2026-2027 school year.  If you do not complete this form by the April 1 deadline, you will have an opportunity to complete the intent form by November 1s for the Spring 2027 semester only.
