Eligibility

• General education students K-12 & their families

• Special education students may be eligible for services depending on circumstances.

• Priority will be given to students who are not receiving counseling services from another provider.

Referral Form

This form is monitored regularly and will be forwarded to the consultant in your district. Please fill out as much of the form as possible.

As screeners are implemented, data from screeners will be used to determine eligibility and priority for services for student referrals.

Termination of Services

Services will be terminated when original and current treatment plan goals are met, the school year ends, when a child is referred to a higher level of care with a community provider and appropriate transition has occurred, and/or per parent’s request.
