Immunization & Physical Requirements 

HEALTH Documents

Gather the necessary health documents listed below.  Heath interviews will be scheduled with the nurses in early April to review your student’s health history and create his/her school health record.  Nurses will not meet unless required paperwork is present.  Minimum of immunization record and last physical even if it is dated before 8/31/24. Children may not enter school until a School Nurse has verified that all medical requirements have been met. 

1.    HEALTH SURVEY -   Kindergarten 2025-2026 Health Survey

2.    Proof of a Physical Exam completed after August 31st , 2024.

You will receive an email with sign up genius link to make an appointment for the health interview with the nurses in the upcoming weeks. In the meantime please gather CURRENT physical and immunization record, please upload to your family connection account.

Record must indicate month/day/ year of the following:

- Diphtheria, Tetanus, Pertussis (Dtap) – 5 doses

- Polio – 4 doses

- Measles, Mumps and Rubella (MMR) – 2 doses

- Hepatitis B – 3 doses

- Varicella (Chicken Pox Vaccine)- 2 doses (or verification from your physician that he/she had the disease)

- Proof of lead screening

3.    Certification of Vision Screening including Stereopsis conducted by your primary care physician or an eye doctor dated after August 31, 2024.

4.    Risk Assessment for tuberculosis and TB test if indicated by your physician.
