Research your disease. Use your textbooks, notes from other classes, as well as appropriate resources online.
You will want information on the reservoir, the portal of exit, the mode of transmission (with examples), the portal of entry, and the susceptible host.
You will also want information on how we can control the transmission of your selected infectious disease

You will find your disease/organism in the drop-down menu and you will have your own page to input your information. Make sure to put your name in there somewhere.
P.S. Check out the Rabies page for an example. Go as fancy or simple as you wish. Have some fun with it.
