Consent Forms

These consent forms are required for participation in the MLTM After-School Program. Students must assent to participating in research, their parent(s)/guardian(s) must consent that their child will be participating in research, and the trained educators (facilitators) running the programs must consent that information about the sessions must be reported by them as part of the research project.

All available forms are provided below to view. First is the Student Assent form, second is the Parent Consent form, and last is the Facilitator Consent form.

To download a form, on a computer, hover your cursor over the desired form, click the icon in the top-right corner of that small window, and click "Pop-out". This will open the document in another window. In that window, click the vertical ellipsis (...) and select download.
