• All athletes are required to have a current physical completed on or after May 1, 2020 for the 2020-2021 athletic year. The form required may be downloaded at this web address:

• All athletes must also have a copy of current proof of insurance.
• Without these documents students will be unable to participate in any athletic tryout, practice, or event.
