Covid Waiver
Medical Release form (please bring insurance information to tryouts if this is not completed)
USAV Badger Region fall/tryout membership or full membership (bring your Membership card (or a photo/screenshot of it) to tryouts).
If you quality for Free/Reduced Lunch we will do this for you at registration prior to tryouts (come early!)
