Outlined below you'll find information regarding the required contracts and evaluations for completing an internship for credit.
Internship Contracts 1 & 2 can now be completed via Google Forms.
Due at the end of the 3 week withdrawal period each semester. To officially register and receive credit, this contract must be completed and filed before the internship begins.
Please NOTE: In order to complete this form successfully, you will need to have the following information readily available and have communicated with your Faculty Supervisor and Site Supervisor to obtain the information:
Your GPA
Academic program granting you internship credit (the major/minor you want to earn credit under)
The internship course codes: http://shorturl.at/jnpz0
Academic Advisor's name & their email address
Site Supervisor's name
Site Supervisor's contact information (address, phone, & email)
Brief description of the duties and responsibilities you'll take on at your internship site
Number of hours you will work at your internship across the semester/term
Number of credits you'd like to earn
Faculty Supervisor (the faculty member within your major/minor granting you credit that will oversee the academic component of your internship) & their email address
Your Faculty Supervisor's responsibilities and the number of times they will complete each responsibility
Your internship course assignments assigned by your Faculty Supervisor
To be completed by the student in consultation with both the site supervisor and faculty supervisor and completed within three (3) weeks after the internship begins.
To complete the Google Form Internship Contract 2, please be sure to have the following information readily available:
Site Supervisor's name
Site Supervisor's contact information (address, phone, & email)
Brief description of the duties and responsibilities you'll take on at your internship site
Academic program granting you internship credit
Faculty Supervisor's Name & Email
Which of the 8 career-readiness competencies you want to develop while at your internship
