Who should use this form?
For students, coaches, parents, and staff that help organize activities and/or fundraisers for Livermore High School. This form must be used by all Livermore High School groups that want to host an activity/fundraiser. That list of groups includes but may not be limited to clubs, athletics, FFA, music, PTSA/SSGN, etc.
Why should I use this form?
The form will allow you to reserve on-campus locations, request custodial services, and register fundraisers on and off campus. The purpose of this process is to avoid scheduling conflicts that are located in the same space or similar activities/fundraisers running at the same time.
What happens between when I turn in the form and when I receive approval?
When the form is submitted, it goes through a series of checks before your activity/fundraiser receives approval. This process takes time, so you must submit your form at least 2 weeks prior to your event. We highly recommend at least three weeks! It is also important that you provide thorough information so it can be approved in a timely fashion.
1) The Activities Office will check for location availability and conflicts with other events.
2) Students in ASB leadership will evaluate the activity/fundraiser and determine ASB approval.
3) Mr. Martinez (ASB Administrator) will evaluate for administrative approval.
4) The Activities Office will place the event on calendars, digital advertising, and email approvals.
Important notes!
ASB meetings are on Tuesday or Wednesday mornings (depending if it is a green or gold day due to when ASB meets for second period). Paper Activities Requests submissions must be in by 1pm on Mondayto the Activities Office in order to have a chance of being included in that Tuesday OR Wednesday's meeting (depending on a green/gold favored week). If there are any issues or missing information, we will hold it for the next week.
It is important that no advertising or commitments are made before you receive final approval. Submission does not guarantee approval.
Guidelines & Procedures
An Activities Request form must be filled out for the use of any of the facilities on campus and any time you are selling anything whether on campus or off campus.
1. All requests must be submitted to the Activities Office at least 2 weeks prior to event date. Please be specific about your needs, ie. if you need the Student Union and also would like use of the Lecture Room, please request both. The contact information (phone and email) must be the advisor's information, not the student's information. This also ensures the advisor's approval and that the advisor will be sent the confirmation or denial email.
2. The Activity Request form must be approved before any flyer, website description, Daily Gram/CBNN announcement or marquee ad is processed. Please note: Flyers cannot be distributed until approved by the administrator in charge. If this is a fundraiser you must submit a fundraiser request as well.
3. Once the Activity Request is approved, a confirmation email will be sent. If denied, a reason will be sent. You can re-apply if you just needed to make corrections.
4. The "advisor in charge" of the event (as filled out on the form) is responsible for set up, clean up, and gaining access to the facility:
• The facility used for the event must be placed back in its original state, i.e. tables and chairs put away, trash picked up, etc.
• Please indicate special needs for custodians on form and the number of hours needed. Please note: Weekend events require a minimum amount of custodial hours and compensation
• For facility access prior to event, please coordinate with an Administrator, Custodian, gain Sonitrol codes, or get keys for access during non-business (non-school) hours.
Is responsible for the handling of cash box and cash box forms (3).
After the Administrator approves the request, it is the Advisor's responsibility to make all necessary arrangements with contacts at least 48 hours prior to the approved event.
