Forms for over the counter medications, routine medication administration as well as asthma, diabetes, severe food allergies and seizure disorders are found on the LISD website under the Parents tab at the bottom of the page. This takes you to the departments pages where you will click on Student Health Services and then Parent forms for Student Health Care. Parents will need to enter through the administration office to log in to Lobby Guard and have a Covid 19 screening including temperature check and answer questions prior to entering the office area. Your student will be brought to the office or if you need to visit the nurse, you will be escorted to the nurse's office. Any other visits will be by appointment only.
Masks, social distancing and sanitizing are the first line of defense against Covid 19. Temperatures will be taken during the first period class by their teacher and will be sent to the nurse if their temperature is 100.4 or more. If your student either tests positive for Covid 19 or has had close contact with someone who is positive, please contact the school and your information will be forwarded to the Director of Health Services who will contact you to determine a plan of action.
