CHILDREN THREE AND UP For Eligibility Determination for children three and up, please contact Intake at the Clermont County Board of Developmental Disabilities by e-mail or by phone.
Eligibility for the Clermont County Board of Developmental Disabilities services is determined based on standard eligibility criteria.
Children under three with one developmental delay or at risk of one developmental delay maybe eligible for services.
Children three to six are eligible if there are two developmental delays or if the child has an established risk.
Children six through 15 are eligible if there are three or more substantial functional limitations are determined through the Ohio Children's Eligibility Determination Instrument (COEDI).
Individuals 16 or over are eligible if there are three or more substantial functional limitations as determined through the Ohio Eligibility Determination Instrument (OEDI).
Upon determination of eligibility, referrals for appropriate services are made. If an individual is determined ineligible, alternative services that meet the individual’s needs are identified and referrals are made to assist the individual in obtaining those services. Eligibility must be determined prior to enrollment in the Clermont County Board of Developmental Disabilities services, but is not necessary for information and referral.
The Ohio Department of Developmental Disabilities (ODODD) must certify Agencies and Individuals that provide Supported Living (SL) and Home and Community Based Waivers (HCBS) Individual Options Waivers (IO) and Level One Waivers.
Provider Certification is the focal point for evaluating the application to determine if an applicant meets the standards that govern the SL and Waivers.
Applications are reviewed on an individual basis and must include specific qualifications and/or documentation to support how the applicant meets or exceeds the service requirements. Providers certified to deliver Medicaid services will receive a Medicaid number necessary to obtain reimbursement.
An Agency or Individual is prohibited from proving any service until certification is obtained from ODODD.
