Steps for Enrolling a Student

You may register for the H. Clarke Powers Extended Day program during hours of operation or during the school day at the school site.

PARENT/LEGAL GUARDIAN ONLY MUST COMPLETE THE FOLLOWING PRIOR TO CHILD’S ATTENDANCE.

1. One-Registration/Release form per child.

*It is imperative that emergency phone numbers (i.e., those persons responsible for picking up your child in an emergency) be CURRENT, LOCAL and UPDATED regularly.

*Failure to provide a minimum of two emergency numbers will jeopardize continued child care.

2. At time of enrollment, the enrolling parent(s) will

indicate who is authorized:

1) to make changes to the Student Enrollment/Emergency form, and/or

2) to have access to their child(ren).

Emergency form changes will only be accommodated by those person(s) specifically identified on the Enrollment/Emergency form

(e.g., mother, father, legal guardian, etc.)

3. Medical Assistance Waiver for medication (optional).

It is the PARENT’S responsibility to deliver medication and authorized physicians’ form to H. Clarke Powers Extended Day Program.

4. Waivers are required for any reason that a child enters or leaves Extended Day during program hours without a parent or guardian’s “sign-in/out”.

5. Only parent or legal guardian may sign emergency forms and registration forms.

6. Parent rights. Each custodial parent has full right to access all student information.

In the case of divorce or separation: Loomis USD Extended Day will recognize the contracting parent as the person responsible for ALL payment, child emergency information, etc.

Court orders splitting child care costs, (i.e., 50/50) are the responsibility of the contracting parent to resolve. H. Clarke Powers Extended Day Program invoices will not be sent to non-contracting parents.

The non-contracting parent may submit a written request and has a right to copies of student records related to their child(ren). Upon written request, the contracting parent will be notified that we will release the requested information with 2-4 weeks from the date of the request for the current school year.

The only exception to the above statement would be in the event of a court document restraining child contact and/or emergency information with Loomis USD listed as a “named party.”

Parents who call our office to request copies of previous bills, calendars, etc., may be charged a fee depending on the request.
