A "Release of Information" form , once signed and dated, authorizes one party (example: out of school therapist) to consult with another party (example: school social worker). If you would like any Longwood staff member to be able to converse/consult about your child with your child's doctor, therapist, occupational therapist, etc... you must complete this form, and return it to the Longwood staff member who you wish to speak to an out side source/provider.
