Screenings

*** Scheduled screenings will be the week of November 17th - 21st.   ***

*** Grade 5 postural screening will be on Wednesday November 19, 2025 ***

The following screenings are done here at Center School.  These screenings are mandated by the Department of Public Health in Massachusetts.

• Vision screening:  done on students in Grades K thru Grade 5.  If your child sees an eye specialist and you want to opt out of this screening, please send a letter to the nurse as well as your child's most recent eye exam from the specialist.  

• Stereopsis screening:  done on students in Grades K thru Grade 3.  Stereopsis shows how well both eyes work together.

• Hearing screening:  done on Grades K thru Grade 3.

**Vision, stereopsis, and hearing screenings are done by the nurse only if the physician has not checked off on the physical form that it was done.

• Height & Weight (BMI) screening:  done on Grades 1 and  4. Parents have the option to opt out of this screening by sending in a signed letter to the nurse. 

• Scoliosis screening:  done only in Grade 5.  Parents have the option to opt out of this screening by sending in a letter to the nurse. Also, the child needs a note from their physician noting that scoliosis screening was done and the result.
