Confidential Medical Information Form: This form needs to be completed yearly. Please make sure to update physician and dentist information.
Allergy and Anaphylaxis Emergency Plan: This form is used and must be completed for those children that have allergies and use an Epi-pen. Must be signed by both parent and physician.
Asthma Action Plan: This form is used and must be completed for those children that have a diagnosis of Asthma and use an inhaler. It should be completed by the physician.

MEDICATION CONSENT FORM - This form is required for all medications that are being administered during the school day; this includes daily medications and PRN medications.
CONTACT INFORMATION FORM - This form is required for all students taking medications at school, whether it is a daily medication or a PRN medication such as an epipen or inhaler. This information ensures that you will be able to be notified if there is an emergency.
