District Policy for Physical Education Medical Notes
If your child does not participate, a parent’s written request for an excuse will be accepted for up to a week. After that a written request is needed from a health care provider. If a parent continues to write a note for the same injury for over two weeks, staff will notify school nurses to assess the injury and work with the family. All doctor’s notes need to have a time frame on the restrictions of that injury. If the doctor’s note does not have a time frame, then the school nurse will work with the family and physician for clarification.
A student returning to school with sutures, ace bandage, casts, splints, crutches, can, walker or a wheelchair must have the written permission of a health care provider to attend school that includes any recommendations and /or restrictions related to PE, mobility and safety.
A student returning to school following a serious injury, surgery or other hospitalization must have written permission by the health care provider to attend school including recommendations regarding physical activity.
An excuse (less than 10 weeks) from a physical education class may be granted to a student who is unable to participate in regular or modified curriculum for a temporary period of time due to illness or injury.
