Documents required for existing family registrations, please provide:

1. Information form for entering kindergarteners

2. Student Health Information Form 

3. Your child's most recent physical and immunization record

4. Certificate of Residency

If you have moved since the last time you registered with Central Registration, please follow the instructions below:

    Contact the Central Registration Office at: LPScentralregistration@gmail.com. 

You DO NOT need to re-register online, however, you will need to provide updated proofs of residency, as well as the following documents:
