Counselor Statement Request
The Student Questionnaire, Parent/Guardian Questionnaire, and Activity Resume are used to write your counselor statement for colleges, scholarships, and other programs.
Please make a copy of the Student Questionnaire, and review the information regarding the Activity Resume.
Complete the Student Questionnaire and create an Activity Resume (using the suggested format) that best represents you, and share both with your School Counselor.
Encourage your Parent/Guardian to make a copy of the Parent/Guardian Questionnaire to share with your School Counselor.

If you have a Google account, please make a copy and share the completed questionnaire with your child's school counselor.

Teacher Letter of Recommendation Request
The Teacher Letter of Recommendation Request questionnaire will provide information to assist in writing a teacher letter of recommendation. Your input is valued and appreciated. In responding please consider what you think is most important to include in your letter.
After you have spoken to your teachers to request recommendations, please make a copy of the questionnaire and complete one for each teacher for whom you are requesting a teacher recommendation.
