VOLUNTEER DRIVER FORMS:

In order to drive students, ALL DRIVERS must have completed the VOLUNTEER DRIVER FORMS.

Follow the links below to access the online forms.

Volunteer Driver Forms need to be completed every year to make sure all drivers are cleared to transport students.

Once completed, please PRINT or EMAIL the completed forms to your coach prior to driving.

TOTAL FORMS NEEDED TO BE ABLE TO VOLUNTEER:

1. School District #68 Volunteer form

2. Criminal Record Check (this step can be done online - it will register with District Database - good for 5 years)

3. Volunteer Driver form

4. Driver's Abstract (this step can be done online and printed)
