CONSENT FORMS
The following Consent Forms include information to review with your students along with signature requirements. Teachers, the first two pages of the document before the actual consent form can be used to teach or review digital citizenship opportunities with your students.
Fillable PDF Consent Forms:
AP334-1 is for Google Workspace for Education - for all students using learn34.com
AP334-2 (blank form) is for Additional Digital Applications/Sites - Individual teachers must use this consent form which lists all potential applications/sites they intend to use during the school year. Click this link for a sample pre-filled form in Google that can be edited. (File>Make a Copy)
AP334-3 is for BYOD (Bring Your Own Device- Middle and Secondary)
Fillable PDF Tips:
On a computer, after you click on the link, the PDF file will probably open in your web browser. The next step is to download the PDF file to your computer. Do this by looking for the download symbol or possibly right clicking to find the download option. Once the PDF file is on your computer open it with a program such as Acrobat Reader or Preview (Mac). Then you can fill in the form and email it back to school.
Legislation Information:
March 2020 - The Freedom of Information and Protection of Privacy (FIPPA) legislation has been temporarily relaxed however the district will continue with the consent form process so parents are aware of online apps and services that involve private student data and so that proper documentation is in place if/when the FIPPA legislation reverts after June 30.
Regarding Google for Education - If consent is not in place, alternative learning opportunities must be provided by the teacher; this is legally required by the Freedom of Information and Protection of Privacy (FIPPA) legislation
