Your health is of concern to your coach and every member of your team. In the event you feel ill or injured, report to your Athletic Trainer or School Nurse at the Middle School as soon as possible. They can assess you and guide you on going to the doctor as well as inform you of any paperwork that you will need to take with you.
If you go to the doctor before seeing the Athletic Trainer or School Nurse at the Middle School, you must bring back WRITTEN instructions from the doctor as to your care.
An injury that could possibly hamper your ability to participate must be reported to the Athletic Trainer as soon as possible. Specific instructions regarding treatment by the Athletic Trainer will be given to you at the start of your season. If the Athletic Trainer sends you to the doctor, the Athletic Trainer will give you a form to take with you, if you forget/lose the form be sure that you ask for and bring back WRITTEN instructions from the doctor as to your care. A RETURN TO SCHOOL OR PE note will not suffice. The note needs to be specific in order for you to return to athletics.
If an athlete is too ill or injured to practice then he/she must report to the Athletic Trainer.
If you go to the doctor on your own, or the AT sends you to the doctor you MUST bring a written note (see physicial referal form below) from them specifying the following:
What is injured
What limitations they have for workouts
When they will be release and/or if they have to return to the doctor
High School Treatment Hours:
6:30am-7:45am
During your athletic period
After School until 5:30pm
All athletes are encouraged to receive treatment as often as possible, without hindering academics.
We are CLOSED DURING DEN AND LUNCH PERIODS
Ascension has assigned an athletic trainer to be at the Middle School 3 days a week.
Tuesday Afternoons
Wednesday Mornings
Friday Afternoons
If an injury cannot wait until the Ascension trainer is on campus, the athletes are more than welcome to see the high school ATs.
Treatments at the high school are before school from 6:30-8:30 for middle school athletes.
If you go to the doctor on your own, or the AT sends you to the doctor you MUST bring a written note (see physican refferal form below) from them specifying the following:
What is injured
What limitations they have for workouts
When they will be release and/or if they have to return to the doctor
Take this to the doctor with you so we know A) what you can/cant do during workouts, B) what the doctor would like us to do in the training room with you
Return to Play and Return to Learn forms are both needed when sustaining a concussion. These will need to be signed by a doctor in order to be released to us for protocol.
