The Rank One forms are different then the ones required for Leander ISD Fine Arts. If your child competes in both Fine Arts and Athletics a physical must be turned into both groups and forms completed on both Rank One sites.

Return to Play and Return to Learn forms are both needed when sustaining a concussion. These will need to be signed by a doctor in order to be released to us for protocol.
