If you graduated from Lakeview High School more than a year ago, additional information is required in order to process high school transcript requests. Please call the Guidance Office at 330-638-5874. A $5.00 transcript fee is required (effective 1/1/2020). Only one diploma is ever issued. We do not have copies of diplomas.

If you need your high school transcripts sent to an institution, you must complete the Transcript Request Form.
If you are using the Common App, you only need to submit this request form one time, listing one school.
Seniors
Seniors
A one-time $10.00 transcript fee is charged in August of your senior year. This fee is to help defray the cost of senior related materials and mailings.
PLEASE NOTE: in most cases, transcripts must be sent directly from the Guidance Office to the requesting institution. PLEASE ALLOW 5-10 days notice for postmark deadlines. Transcripts will not be sent out the same day as they are requested.
