• Official identify document as a PDF, JPEG, JPG, PNG ( Passport, Birth Certificate, or a State Issued Driver's License, Learner's Permit, or ID)

• All exam series student has taken

  • Make sure to enter the month and year (Ex. June 2024)

  • Center Number or Name (UX777 South Lake High School)
