Steps to complete this form:
Make sure you complete your hours for a not-for-profit organization (i.e. a school, a charity, local festivals, Terry Fox Run, etc...)
Fill out the details of your services hours on the sheet.
Make sure your supervisor from the not-for-profit activity signs this form.
Make sure your parent/guardian signs this form.
Once you have completed the above steps, please drop off the form in Guidance.

40 Community Service Hours are required for
ALL students in Grade 9 to Grade 12.

Students must complete 40 Community Service Hours to receive their OSSD (diploma).
These are often referred to as "volunteer" hours, when in fact they must be COMMUNITY SERVICE HOURS.
These hours must be UNPAID.
These hours must be OUTSIDE of REGULAR SCHOOL HOURS.
They cannot be for private or profit businesses, companies, or people.
If you are unsure whether your hours are APPROVED, please see GUIDANCE or your PRINCIPAL to get the "PRE-APPROVED" box signed off on your record sheet PRIOR to BEGINNING those hours.
Please see the lists below for examples of both APPROVED and UNACCEPTABLE Community Service Hours.
