Please make sure to complete all required Health Action Plan(s) for your student before school begins.
If medication is required at school, a new Medication Authorization form  must also be completed prior to the start of each school year for any prescription or over-the-counter medication that will be administered or available at school.
A new Health Action Plan is required each school year for students with asthma, allergies, seizures, or diabetes.
Action Plan(s) can be faxed to 920-459-2930, attention Kelsey Wolf, District Nurse OR scanned in and emailed to wolfk@kohler.k12.wi.us
