Part of our responsibility in caring for peoples’ well-being and safety at Knox is documenting when incidents or accidents occur. This is done using an Incident Report Form and is a procedure that every volunteer must be familiar with.

An Incident Report Form needs to be completed anytime one of the following occurs:

A physical injury (beyond general first aid like providing a band-aid)
Mental health distress
Harassing behaviour (physical, verbal, sexual, etc)
Situations in which someone felt unsafe
Significant damage to an individual’s property or the church’s property
Take a few moments to read Knox’s Incident Reporting policy, which explains in detail who should complete the report, when it should be completed, and how it can be submitted for review.
Printed copies of the form are available in the front office closet, in the kitchen first aid kits, and always accessible on this site.
