Health requirments
All incoming kindergarten and new students to Simonds are required to have a copy of a recent physical exam (dated within 1 year) on file with the health office and a copy of NH required immunizations for school entry.
Please make sure your student's immunizations are up to date and provide a current copy to the school nurse. You will be notified if they are not.
NH Required Immunizations for School Entry 2023-2024
(per RSA 200:381)
DTaP:4-5 doses (diphtheria, tetanus, pertussis vaccine)last dose given after 4th birthday
Polio: 3-4 doses, last given after 4th birthday and last doses separated by 6 months.
Hepatitis B: 3 doses
MMR: 2 doses, first must be on or after 1st birthday
Varicella: 2 doses
Children must have proof of all required immunizations or valid exemptions in order to attend school.
Please inform me of any medical conditions, health concerns, or allergies (especially life-threatening) that may impact your child at school. Please also complete and return the health history paperwork to the health office. Strict confidentiality is maintained for student health information.
If your child takes a daily medication that needs to be given at school,
please have the physician fill out a medication administration form.
If your child has asthma and will leave an inhaler at school, please also have a physician complete an asthma action plan
Some medical diagnoses may also require an Emergency Action Plan signed
by a physician (ex: Seizure disorder, Life-threatening allergies, Diabetes, etc).
All student medications must be in the original labeled container.
Over the counter medications (cold medicines, allergy meds etc) can be administered if sent in the original bottle with a permission note signed by parent/guardian. OTC meds will only be given per bottle instructions unless accompanied by physician note.
